At Rejuvalife Vitality Institute, we are dedicated to excellent patient care and look forward to serving you.
Your consultation is designed to meet and discuss your health, cosmetic and/or skin care needs, outline the procedure or treatment options, and inform you of the fees. Every patient is a unique, special individual and every treatment plan is tailored by Dr. Andre Berger to address your specific needs and goals. The initial consultation appointment is critical to both you and Dr. Berger since it is during this time that you both come to clearly understand each other – the real issues, and define realistic goals to achieve the desired results. This appointment requires extra time, so please plan accordingly.
OUT OF TOWN CONSULTATIONS
Patients from out of town routinely take advantage of our telephone or e-mail “pre-consultations.” Be aware that this option is not limited just to patients traveling from out of town to our office, but is also available to patients from the local Beverly Hills and Los Angeles area as well. If you’re contemplating body sculpting or age management, please request a consultation with Dr. Berger or call our office at 310.856.9173 to get pre-consultation, preliminary information about the treatment plan, the costs involved, and schedule availability. We can happily supply that information via telephone call or e-mail at your convenience.
Tracy is our Patient Coordinator and is the person patients work with the most from the first phone call and throughout your experience at Rejuvalife. Tracy schedules all of Dr. Berger’s surgical and initial anti-aging appointments and manages the process for patients. She’s not just a front office person, but is an active patient advocate and source of information for new and experienced patients alike. Her gentle spirit, office experience and knowledge mirror Dr. Berger’s own practice and care philosophy.
Please come 15 minutes early to your schedule appointment to fill out the necessary registration paperwork needed. Our staff is here to help you through the process should you have any questions.
All will agree, time is our most valuable asset. We ask for a 2 day change or cancellation notice to avoid a cancellation fee of $150. We understand things come up and we will do all we can to be flexible and understanding should a conflict arise. We in turn ask for your respect and consideration for our policies and scheduling. Please arrive 15 minutes prior to your scheduled appointment to complete required forms. This will help insure our schedule runs smoothly.
One-hour free parking is available about a half block away from our office or across the street at Rite Aid. You may also utilize metered street parking and valet in the rear of our building, off Canon Drive, just south of Brighton Way.
FORMS OF PAYMENT
Payment and balances for all services is required at the time of service. We accept Visa, MasterCard and cash. We do not accept personal checks unless it is two weeks prior to your treatment date.
We are not an insurance provider, however, for medical services or labwork that may be covered by insurance, we can give you a superbill that can be submitted to your insurance company and they may reimburse any monies directly to you.
PAYMENT PLANS AND FINANCING OPTIONS
Regardless of your financial situation, you deserve to look at feel your best! That’s why we offer financing plans for a variety of budgets. Our patient coordinator Tracy, is happy to discuss options available through our office and from CareCredit.
OUT OF TOWN PATIENTS
Dr. Berger is renowned internationally and patients travel from around the globe to entrust themselves to his expert care. Rejuvalife Vitality Institute is located in the heart of Beverly Hills with excellent accommodations, restaurants, shopping and celebrity watching all around us. Dr. Berger’s clients have found Beverly Hills to be an enjoyable and relaxing area both – before and after their appointments. Our multi-lingual staff is known for their ability to make all of our clients feel right at home, while away from home by providing unlimited, caring support to enhance your personalized treatment plan. Your initial visit with Dr. Berger is critical for both you and him, your follow-up appointments are equally as important. Therefore, we do everything we can to ensure that all of your visits (appointments) are both convenient and positive experiences.
For patients traveling by air, the two most convenient airports are the Los Angeles International Airport in Los Angeles, California, and the Bob Hope Airport in Burbank, California.
